Integrity takes a consumer-centered approach to Care Plans. Our Vision is to create a new normal that celebrates and promotes inclusivity and integration, so that people with exceptional needs can feel fully accepted within their communities and support networks.
Integrity partners with the Wisconsin Family Care programs to provide 1-2 bed Adult Family Homes (AFH) throughout Southern Wisconsin. In these instances, Integrity provides the housing and the services. We partner with several Managed Care Organizations to certify these homes in accordance with Wisconsin regulations. We are currently partnering with MyChoice Wisconsin, IRIS, Community Care, Lakeland and several counties as well. We also provide Supportive Home Care, in which the resident has their own housing, and we deliver services to them in their residence.
Integrity has really put in place an amazing team of staff that really are a good fit our daughter and are able to provide her the support that she needs.
Integrity appears to be very capable and successful in providing him with a safe environment. Additionally, I wanted to pass along the case manager, informed me you send over weekly behavioral charting which is so helpful regarding authorizations and keeping necessary care and supervision in place. I am so pleased to see excellent communication within a team, as that is not always the case!
I want to thank you for being such a bright spark in our daughter’s life and program. Your caring for her (and her staff) is making such a huge difference. It has often been hard for others to see past her behaviors to the wonderful person and spirit she is, and you have been able to do that. You are a gift.
I know how much you each did for him and I feel like you saved him both when he needed placement and then through his many challenges, particularly while he was on hospice and was allowed to eat and drink in an effort to give him more life satisfaction and quality of life. I can only hope that he is again treated with respect and given the love and attention he has enjoyed with you. You embraced him when he was shunned by others and got him through very difficult times. He will not forget all you have done for him, nor will I.
